THE PROBLEM:

  • DEATHS ~ According to the Institute of Medicine 1999 report, To Err Is Human, each year between 44,000 and 98,000 people die from preventable medical mistakes made in hospitals

  • INJURY ~ in addition, one million preventable medication mistakes occur annually.

THE SOLUTION:

Reported are these facts:

  • If three Leapfrog standards were implemented in all U.S. urban hospitals (ICU staffing; CPOE; higher-performing hospitals for high-risk procedures), the nation could save up to 57,000 lives, avoid as many as 3 million adverse drug events and save up to $12 billion in health care costs each year.   • A study in the Joint Commission Journal on Quality and Patient Safety found that hospitals that performed adequately on the Leapfrog Survey have lower mortality rates and better quality of care than hospitals that declined.

CONSUMERS: The Leapfrog Group'S Four Key Questions
The four key questions consumers should be asking when they are making decisions about their hospital care are:

  • Does the hospital use a computerized physician order entry system (CPOE system)?
  • Does the hospital have a high rate of good results or extensive experience with the specific procedures or diagnoses?
  • Is the Intensive Care Unit (ICU) adequately staffed by physicians and other caregivers who are specifically trained in intensive care?
  • Does the hospital have a high Leapfrog Quality Index? Thismeans the hospital has put in place up to 27 practices known to reduce preventable medical mistakes. 

WHAT'S UNIQUE ABOUT THE LEAPFROG HOSPITAL SURVEY?

  • The Survey is the only public source on adoption of CPOE, hospital efficiency, adoption of endorsed safe practices, survival predictors, presence of an ICU intensivist, and risk-adjusted mortality rates for certain high-risk procedures.
  • The Survey is the private sector’s consensus public reporting initiative.
  • The Survey questions are consistent with measures used by organizations such as The Joint Commission, National Quality Forum (NQF) and Centers for Medicare and Medicaid Services (CMS).
